The first time a doctor used the word herniated in front of me, I pictured a tyre with a bulge in the sidewall. You know the sort. Once a tyre goes like that, it is finished, and you drive on it with one eye on the road and one on your wallet. That is roughly what most of us hear in the word. The worse it looks, the better it clearsA meta-analysis published in Pain Physician pooled 31 studies covering 2,233 patients whose lumbar disc herniations were treated without surgery, and found spontaneous resorption in about two thirds of them. A systematic review in Clinical Rehabilitation then sorted herniations by severity and found regression rates of 13 percent for a simple bulge, 41 percent for a protrusion, 70 percent for an extrusion, and 96 percent where a fragment had broken free altogether. A 2023 meta-analysis combining further data reproduced the same ladder. The leading explanation is that material which has escaped furthest is most exposed to the blood supply and gets cleared away like any other foreign matter, while the fragment also dehydrates and shrinks. The timing is the catch. Large herniations in these reports resolved anywhere between three and twenty-one months. These are imaging findings across groups of people. They are a reason for hope and a reason to ask good questions, not a substitute for your own doctor’s judgment. |
